We DO NOT provide superbills, we only provide receipts for our services.

​CLINIC VISIT

$150 - Established patient, rapid testing and/or sick & urgent visit + any additional labs or services.

​

$​200 - New patient visit, annual exam, well visit exam, work physical, in-depth visit, sports physical, multiple issues, lab reviews, + any additional labs, referrals for mammogram, colonoscopy, dermatology, other referrals and requests.​

​TELEMEDICINE & TELEWELLNESS VISIT: 

$200 includes a telemedicine visit, consults, lab/chart reviews, any referrals necessary, lab order, and medications the same day.  Medications, Vitamins, Hormones and prescriptions not included in your visit however, they are ordered for you and called in to your pharmacy of choice.  You will pay for your own medications at your pharmacy.​

We can submit your insurance card to LabCorp OR any other lab.  We can also send you to any lab you desire for a blood draw/labs.   We do charge for a blood draw (venipuncture) and specimen handling fee.
